To help us understand the scope of your relocation, please provide as much of the following information as possible:

  • Clinic or laboratory name
  • Current laboratory location
  • New laboratory location
  • Preferred relocation or service date
  • Alternate available dates
  • Type of laboratory being relocated
  • Approximate move timeline
  • Equipment being moved
  • Equipment manufacturer, model, and serial numbers if available
  • Whether equipment will need disconnection, reconnection, PM, calibration, or validation support
  • Whether cryostorage tanks or monitoring systems are involved
  • Whether equipment is being moved by your team, a moving company, or another vendor
  • On-site contact name and phone number
  • Any access, parking, loading dock, security, or badge requirements
  • Any deadlines related to inspection, opening date, patient care, or laboratory restart

Before the Move

After the Move
